Please click on a tournament below to view and print out the corresponding host form.  If you are a traveling team (not from PA), this form must be used to obtain your state approved travel papers.  Your state approved travel papers can be provided by your league or club president and typicaly come as 3 copies. One copy for your records.  One copy for us.  And one copy for the state office that you are traveling from.   For more information click here.
